IMNSHO, there are a few factors:
* Welcome to Capitalism, leave your soul at the door.
* Inflation
* The mathematician in me thinks this is obvious: companies want to maximize p*u where p is profit per unit and u is units sold.
* Corporations don't want to pay upfront costs (real or perceived) to make changes to development processes
* Developers are set in there ways and tend to fight changes to processes that have been around 'forever'
